Cargo Record Book
Every ship certified to carry noxious liquid substances in bulk, shall be provided with a Cargo Record Book in the form specified in appendix II of this annex.
At least English, French or Spanish
The Cargo Record Books may be subject to inspection by Port State Control or Government officers while the ship is in its ports or offshore terminals and the officers may make a copy of any entry in the books, and may require the master of the ship to certify that the copy is a true copy of such an entry.
Any copy so made, which has been certified by the master of the ship as a true copy of an entry in the ship's Cargo Book or shall be admissible in any judicial proceedings as evidence of the facts stated in the entry.
The Cargo Record Book shall be kept in such a place as to be readily available for inspection and, except in the case of unmanned ships under tow, shall be kept on board the ship.
It shall be retained for a period of three years after the last entry has been made.
In the event of an accidental discharge of a noxious liquid substance or a mixture containing such a substance or a discharge under the provisions of regulation 3 of this Annex, an entry shall be made in the Cargo Record Book stating the circumstances of, and the reason for, the discharge.
Each entry shall be signed by the officer or officers in charge of the operation concerned and each page shall be signed by the master of the ship.
Entries in Cargo Record Book:
After completion of any operation such as
• Loading
• Unloading
• Internal transfer of cargo
• Prewash,
• Tank cleaning
• Discharge of tank washings into sea
• Discharge of ballast from cargo tanks
• Ventilation
• Accidental discharge
• Exception discharge
When making entries in cargo record book:
- Date
- Operational code and
- item number shall be inserted in the appropriate columns and the required particular shall then be recorded in chronological order
Entries to be made for all categories of cargo after each operation is completed.

[B] Internal transfer of cargo
[C] Unloading of cargo
[D] Mandatory prewash in accordance with the ship's Procedures and Arrangements Manual
[E] Cleaning of cargo tanks except mandatory prewash operations, final wash, ventilation, etc.
[F] Discharge into the sea of tank washings
[G] Ballasting of cargo tanks
[H] Discharge of ballast water from cargo tanks
[I] Accidental or other exceptional discharge
